Job Description:
DescriptionThe Care Consultant will provide guidance and support
to individuals impacted by Alzheimer's disease and other dementia
and respond to disease related questions / situations by providing
guidance, problem-solving, decision-making support, crisis
assistance, and connections to local services. Conduct clinical
assessments and action plans that identify needs and offer
interventions based on those needs, including assistance with
future planning, and in-depth education about the disease and
caregiving. Implement the Alzheimer's Association's program
delivery and growth strategy to reach more people with
volunteer-delivered support group and early-stage programs.
This position is grant funded.

Position Location: Hybrid; Must reside in Herkimer, Jefferson,
Lewis, Oneida, Onondaga, Oswego or St. Lawrence County
Full time based on 37.5 hours per week minimum
Position Grade: 205 Exempt ($60,000 - $64,000)
Reports To: Director, Care & Support
