Job Description:

Professional Medical Practice, P.C., is looking for an Alcohol and Substance Abuse Counselor (CASAC-T, CASAC, CASAC2) to work in the medical department at the Sullivan County Jail located in Monticello, NY.
This position is Monday - Friday during normal business hours 8:00am - 4:30pm. Salary Range $45,000 - $55,000.00
Job Responsibilities:

• Responsible for the clinical monitoring, coordination, and implementation of the behavioral health program in accordance with Company policies and procedures.
• Serve as the liaison between medical and facility staff as it pertains to Medication Assisted Treatment (MAT).
• Engage patient in therapeutic interventions with the goal of addressing substance use related concerns.
• Facilitate group and individual sessions with patients in the MAT program.
• Actively assist with the detection and prevention of patient suicide.
• Document all patient encounters in a timely manner utilizing the SOAPE note format.
• Assist and/or collaborate with facility staff regarding discharge planning for patients in the MAT program, where applicable.
• Facilitate/participate in interdisciplinary team meetings and/or case consultations with staff across disciplines.
• Participate as a member of the Quality Assurance Committee and any sub-committees. Assist in the establishment of Quality Assurance standards and audit criteria.
