About the JobAveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to our patients in the comfort of their own homes.Job SummaryThe LPN will be responsible for delivering and coordinating quality patient care in compliance with physician orders. This includes observing and assessing patient condition, documenting all activities, and reporting changes in condition to the supervisor and/or physician as appropriate.Responsibilities

• Deliver and coordinate quality patient care in compliance with physician orders.
• Continuously observe and assess patient condition and care needs.
• Document all activities, assessments, nursing actions, responses, and coordination of care.
• Participate in and update the nursing care plan.
• Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
