• 24/7 On call support with clinical and scheduling to handle issues outside of office hoursThe Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient.Key Responsibilities:
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate
  • Document all activities, assessments, nursing actions, responses and coordination of care in a timely manner
  • Participate, implement and update the nursing care plan
  • Recognize changes in patient needs and responses requiring intervention, implementing care to prevent risk or reduce riskRequirements:
    • Graduate of an accredited school of nursing
    • Current, unrestricted state license as a Licensed Nurse in the state of practice
    • Current CPR certificationPreferences:
      • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
      • Home health experiencePhysical Requirements:
        • Ability to speak, write, read and understand English
        • Ability to travel
        • Ability to lift 50 pounds
        • Ability to sufficiently reposition patients and move equipment without assistance
